I understand that any false or misleading information provided by me on this application shall be grounds for immediate dismissal at any time during my on-call and at-will employment with HPSS. It is understood that my employment is subject to my satisfactorily meeting the requirements of a security (DOJ) background investigation, to which I hereby consent, and subject to undergoing a medical examination the results of which indicate that I can perform the essential functions of the job, with or without reasonable accommodation, without posing a direct threat to the health or safety of myself or others. I understand that if I do not work my job assignment(s), or if I don't show up for work I will not be paid. Failure to accept positions when offered will result in being placed on the inactive list.
